HA dermal filler girth enhancement is the most evidence-supported minimally invasive penile enlargement option. Average girth increase: 1.5–2.5 cm. Lasts 12–18 months. Autologous fat transfer for girth is permanent but less predictable (resorption rate 20–50%). Ligamentolysis (flaccid length extension) adds 1–3 cm flaccid but does not reliably increase erect length. We do not perform or recommend penile implants for enlargement only, non-medical "stretching" devices promoted as permanent, or silicone injection (banned in Korea and most countries).

Autologous Fat Transfer (Lipofilling)
Patient's own fat is harvested (typically from the abdomen) by small-volume liposuction, processed, and injected into the penile shaft. Permanent but unpredictable — fat resorption rates range 20–50% in published series, and uneven resorption can create contour irregularity requiring revision.

Suspensory Ligament Release (Ligamentolysis)
Surgical release of the suspensory ligament allows the internal portion of the penile shaft to extend forward, adding 1–3 cm of flaccid length. Important caveat: this procedure increases flaccid length; the effect on erect length is inconsistent (typically 0.5–1.5 cm) and not reliably predictable. Requires 4-week sexual activity restriction. Penile rehabilitation exercises required post-op.

Procedures Dr. Kim Does Not Perform or Recommend
- Silicone injection — banned in Korea (MFDS) and most countries; causes chronic granuloma, fibrosis, and severe deformity. No reversal possible.
- PMMA (polymethylmethacrylate) filler — permanent, non-biodegradable; high revision rate; not recommended by EAU guidelines.
- Penile implant for size only — implants are designed for ED management; using them in a man with normal erectile function causes unnecessary surgical risk.
- Marketed "natural enlargement" procedures — any procedure claiming permanent erect length gains above 3 cm without evidence is not offered at Seomyeon L.
If a clinic promises results that appear to significantly exceed published ranges, this warrants scrutiny of the evidence basis. Dr. Kim will provide published literature references for any procedure he recommends.
Candidacy Assessment
Penile enlargement is appropriate when:
- Penile measurements are in the normal range but patient has measurable distress
- Expectations are realistic — HA filler adds girth, not erect length
- No active STI or skin infection at treatment site
- Not seeking enlargement as a substitute for managing ED or PE
Dr. Kim screens for penile dysmorphic disorder (PDD) and unrealistic expectations at consultation. If present, he will recommend psychological support before proceeding — not because he is declining business but because the procedure will not address the underlying concern.
