HA dermal filler girth enhancement is the most evidence-supported minimally invasive penile enlargement option. Average girth increase: 1.5–2.5 cm. Lasts 12–18 months. Autologous fat transfer for girth is permanent but less predictable (resorption rate 20–50%). Ligamentolysis (flaccid length extension) adds 1–3 cm flaccid but does not reliably increase erect length. We do not perform or recommend penile implants for enlargement only, non-medical "stretching" devices promoted as permanent, or silicone injection (banned in Korea and most countries).
Hyaluronic acid filler (same material used in facial aesthetics) is injected into the subcutaneous tissue of the penile shaft under local anaesthesia. The result is immediate and measurable. HA is biocompatible and reversible with hyaluronidase if the outcome is unsatisfactory.

Patient's own fat is harvested (typically from the abdomen) by small-volume liposuction, processed, and injected into the penile shaft. Permanent but unpredictable — fat resorption rates range 20–50% in published series, and uneven resorption can create contour irregularity requiring revision.

Surgical release of the suspensory ligament allows the internal portion of the penile shaft to extend forward, adding 1–3 cm of flaccid length. Important caveat: this procedure increases flaccid length; the effect on erect length is inconsistent (typically 0.5–1.5 cm) and not reliably predictable. Requires 4-week sexual activity restriction. Penile rehabilitation exercises required post-op.
